What is the Proper Identification for Our Baby's Passport?

It is imperative to gather the documents that support the baby’s U.S. citizenship. Use key documents such as a certified birth certificate or foreign birth certificate.

Gather further documents to establish a parental relationship for both parents to the child. This should contain the names of both parents; birth certificate or adoption judgment.

Confirm that each parent has certified proof of identification. Use an intact, government distributed identification such as a previously issued U.S. passport, valid driver’s license, naturalization certificate or military ID is sufficient. Provide proof of a government issued Social Security number.

Make photocopies of the original identification on standard white paper.

What is Required during the Application Process?

Obtain and complete the baby’s passport application. Use Form DS-11: Application for a U.S. Passport. Complete the form online or print and complete a blank form. Write only in black ink.

DO NOT sign the printed form as it must be signed in front of an officer at the Passport Acceptance Facility.

Find a Passport Acceptance Facility (an office authorized to accept the passport application) by communicating with a local post office or use the U.S. State Department's Passport Acceptance Facility online search.

What Pre-Cautionary Measures can I take?

Provide the notarized statement of consent to the acceptance officer should one parent be unable to attend the application submission.

He or she must sign and have the Form DS-3053- Statement of Consent or Special Circumstances notarized.

Locate Passport Acceptance Facility

Take the completed forms (DS-11 Form, original copies of baby’s citizenship, proof of identification, prepared photocopies and Form DS-3053) to the local Passport Acceptance Facility.

If sole custody is the circumstance, present the original copies of baby’s citizenship, your ID, and a completed DS-11 with any supporting documents of primary custody.

Provide the passport fee to the official at the time of application. Make certain that both passport photos are duplicate color photos with a white background. The baby’s head should be clearly visible.

What is the Processing Time for Our Baby's Passport?

It can take four to six weeks for processing and approval of routine applications. Expedited applications can arrive in two to three weeks.
