Explain your reasons for wanting a surrogate/donor:

We are wanting to have a child of our own but due to health reasons I had to have my tubes tied at the age of 23. If I were able to have another child I would have a reversal done but that's not our luck.

How would you best describe yourself(s):

My husband & I have 4 children between the 2 of us. I have 3 boys from a previous marriage ages 13, 11, & 7, & he has a 2 year old daughter from a previous relationship. My husband is a derrek hand in the oilfield & he currently works 7 & 7. I am currently employed as a accounts receivable clerk.
